Insurance Information
It is our practice to provide a transparent look at your financial obligations. Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered.
Our staff will be glad to assist you with your insurance coverage. Please bring your current insurance card with you. If your plan changes, please be sure to let our insurance department know. If an insurance referral is necessary, please bring the completed form with you to your appointment. You will want to check with your insurance provider and determine if a referral from your primary care physician is needed.

We will submit all insurance claims for you, and will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not been paid within 45 days. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance claim form.
We offer a variety of payment options to meet your needs. We accept payments by cash, check, Visa, Mastercard, AMEX, Discover, money order, flex cards, and cashier's checks.
