Background
Baptist Medical Center South and Baptist Medical Center East are engaged in a health equity initiative focused on supporting patients living with Diabetes to have improved A1Cs and health outcomes. At the heart of this initiative are our patients. Each hospital’s Diabetes Patient Advisory Council (D-PAC) will offer patient perspectives to inform and inspire care for patients with Diabetes. Participants will have the opportunity to gain a better understanding of the hospital's Diabetes services offered and become an advocate for Diabetes Care in their community.
Council Structure
Each D-PAC will include six volunteer community members and two hospital team members. Patients with diabetes, who are reflective of the hospital's patient base, will be selected to participate.
Role and Responsibilities
- Participate in quarterly council meetings. One (1) meeting per year will be held in person with virtual capabilities. Remaining meetings will be held virtually to aid in participant access.
- Serve a one-year term, renewable for a total of three (3) terms. Must attend at least three (3) meetings each year to be eligible for an additional term.
- Be an active participant in sharing insights and information about their healthcare experiences in ways that others can learn from them.
- Be an active participant in sharing hospital initiatives with community members.
- Consider serving in a leadership role with the council.
- Interact and work in partnership with others in the group respectfully, with a spirit of collaboration.
