Your First Visit
Thank you for choosing our office for your healthcare needs! We have created a comfortable environment and want you to feel confident in the level of care you receive from our staff and physicians.
What to Expect
It’s important to be prepared for your first visit. Please bring with you:
- Your insurance card(s)
- Your referral form (if applicable)
- Results from blood or urine tests performed by your primary physician
- Any imaging studies you have had done
- A list of all medications you are currently taking, including supplements
- Completed new patient paperwork
- Be prepared to discuss your symptoms
Comprehensive Exam
Depending on your symptoms, your specialist will perform a physical exam as well as a series of tests. These tests, which are generally not painful, will help us accurately diagnose your condition. Working and collaborating with you, we will establish a treatment plan that will best meet your needs. Questions are welcome, and we are happy to discuss any concerns you may have. We believe that candor and open communication are key to a successful outcome. We will discuss our diagnosis and recommendations, any alternative treatments and any symptoms present that may require further treatment.
