SERVICES WE OFFER
- Medical services provided by a patient's chosen attending physician, hospice physicians, and nurse practitioners.
- Skilled nursing visits at individualized frequencies
- 24/7 nurse on call for questions, emergencies, or visits
- Medication support and durable medical equipment as needed
- Social worker services for support
- Home health aide visits (provide assistance with personal care up to 5 times weekly)
- Spiritual support and emotional support for patients and family (including bereavement)
- Volunteer services
WHO WE SERVE
Anyone Who:
- Has been diagnosed with a life-limiting illness that has a life expectancy of six months or less.
- Is no longer planning to focus on curative care or treatment and wants to focus on comfort including pain and symptom control.
