When Should I Consider Baptist Hospice for Myself or a Loved One?
A serious illness can be overwhelming for the entire family. Baptist Hospice is here to help. Anyone can refer a patient to hospice, and the hospice evaluation is free. If you're not sure if you or your loved one might be eligible to receive hospice care, this brief questionnaire might help.
HAVE YOU OR A LOVED ONE?
- Been told by a doctor that life expectancy is limited?
- Been hospitalized or had frequent emergency room visits in the past 6 months?
- Started spending most of the day in a chair or bed?
- Started needing help with one or more of the following activities: bathing, dressing, eating, getting out of bed, walking.
- Started feeling weaker and more tired?
- Started having increased pain or anxiety?
- Experienced unintended weight loss?
- Started feeling weaker and more tired?
- Noticed a shortness of breath, perhaps even while resting?
- Fallen several times over the past 6 months?
Hospice is 100% covered by Medicare for all eligible patients and is covered by most Medicaid and commercial insurance plans.
Life-limiting, end-stage conditions that are treated by hospice (not an all-inclusive list):
- Alzheimer's disease and other forms of dementia
- Cancer
- Chronic obstructive pulmonary disease
- Heart disease
- Kidney failure
- Multiple sclerosis
- Parkinson's disease
- Stroke
